Title

Promoting Physical Activity in Cancer Patients Through a Segmented Blended Care Approach (SAGA) (Research)

Abstract

Early physical activity improves treatment outcomes and recovery in cancer patients, yet only a minority remain sufficiently active. Existing oncology rehabilitation programs often have limited reach. The BeweegSaam blended care pathway promotes physical activity through a scalable, person-centered intervention that is digital where possible and face-to-face where needed. Patients are segmented using the Health Monitor, based on illness acceptance and perceived control, allowing the intervention to be tailored to different needs. This project further develops BeweegSaam for Health Monitor quadrants 2, 3, and 4, focusing respectively on structure and planning, involvement of informal caregivers, and stronger integration within the oncology care pathway through the healthcare team. Using an iterative co-creation process, practice-based evidence is combined with scientific literature to develop an effective and scalable intervention.
