Patient-care advancement with responsive technologies and engagement together (Research)
Abstract
The PARTNER project will offer solutions to support the ideal patient journey for chronic diseases through the health system for optimal care. Data and information collection will be continuous, seamless, and patient-centric. Interdisciplinary health professional decision support for management choices is based on holistic and longitudinal patient data. Extension of data collection beyond the walls of hospitals, emergency departments and operating rooms into their community should be significantly enriched to truly capture the full depth of patient data reflecting their states of wellness and health. Fast collaborative workflows of interpreted and harmonized data representations will not only increase the productivity of the caregivers, but also better justify the patient-centric decisions.
